Your First Visit

Your first visit will require extra time in order to complete your medical history, so please arrive 15 minutes earlier than your scheduled appointment. If you have dental insurance, please bring your insurance information to your visit. When you arrive, check in at the reception desk, and present your insurance information to the receptionist.

You can also save time at your visit by completing your registration forms at home. Simply print our patient forms, fill in the requested information, and bring them to your appointment. You can always complete the forms in our office, too.
